Tip: Save the CE Credit Form to your computer, and after completing an education activity, enter the details of the activity.
Failure to submit the required number of CE Credits will result in your membership changing from Approved Practitioner to Associate Member, however, reasonable consideration will be given on an individual basis;
KEEP all of the ORIGINAL documents for your own records as you may be required to submit a copy for each activity;
The Membership Director will review your CE Credits earned and reserves the right to approve or deny any submitted CE credits based on credibility;
Any questionable CE Credits will be forwarded to the Education Committee for review;
If any CE Credits are questionable, the Membership Director will ask you to submit more information/documentation, for further review;
Members need to be aware of the date of their own Approved Membership Term for CE Credits and must keep a record for their files.
